About HEART ATX
HEART ATX is a team of Community Health Workers (CHWs) that provide services to underserved individuals in the community, Federally Qualified Health Centers, and affordable housing settings. Our CHWs address chronic disease risk factors and social determinants of health (SDoH) and promote COVID-19 vaccination and prevention to build community resiliency in Austin/Travis County.
About Community Health Workers (CHWs)
A community health worker is “a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served “ (source: APHA) Community health workers are sometimes called Promotoras or Community Health Representatives. Because community health workers have lived experience and cultural alignment with the communities they serve, they are able to build trust and be a link between the community and health/ social service providers. CHWs work in many fields including maternal and child health, chronic disease, immunizations, primary care, and more. They have many roles and responsibilities including outreach, community education, resource referral.

Prevent T2 is a results-driven, diabetes prevention program that is designed for people experiencing prediabetes. It features an approach that is proven to prevent or delay type 2 diabetes. Register for Prevent T2
Diabetes Management
Diabetes Empowerment Education Program (DEEP)
DEEP is a 6-series class on how to live well with type 2 diabetes, but is open to everyone. The series gives an overview of diabetes management and monitoring, exercise, nutrition, complications and prevention, and effective communication with health care providers. Register for DEEP.
